There's a third one with Legate Lanius, but I don't remember it.
I have also thought of a way you can display the second scar in a less...flashy fashion. Due to the relaxed muscles from severe brain trauma, the second bullet would do substantially less damaged to relaxed tissue than tensed up tissue. Less damage, smaller scar.
